The AGH Patient Portal website will help those who are healthy by providing preventative tools. Those with chronic illnesses will also benefit from communication tools on the website that enable patients to share and obtain information from their physician securely and easily. Download the

With a secure log in to the AGH patient portal website, patients and their caregivers
will be able to communicate with doctors easily and safely -- at their convenience:
• No more waiting until your appointment – email a question directly to your
physician’s office.
• Taking a medication on an ongoing basis? Request your refills online.
• No need to fill out forms with the same information again and again at each
doctor visit. Enter your health history through the patient portal and it will
be stored in your records.
• Access your own preventative care calendar and track vital health information and test results.
